What types of things will we work on together?

Individual interventions may include up to 12 hours per week of home and community-based support to provide:

Assistance with accessing and navigating community resources

Advocacy within the medical, educational, social service and psychiatric systems

Psycho-education and skills-training to improve independent living

  • Have a current mental health concern (i.e. Depression, Anxiety, problems sleeping, substance abuse concerns, etc.).

  • Stayed overnight in a placement due to mental health concerns (i.e. hospital, incarceration, group home, etc.).

  • Have a need for individualized training in acquiring basic living skills such as symptom management; adherence to psychiatric and medication treatment plans; development and appropriate use of social skills and personal support system; personal hygiene; food preparation; or money management.

  • Been prescribed medication for your mental health concern in the last year.

*If an individual is under the age of 21, they must be in an independent living situation or transferring into one within six months.

*As of Oct 2024, we accept Medicaid through Sentara, United, Humana  and Molina. We're currently working to become credentialed with other MCOs to be able to provide services to more individuals.

What is Mental Health Skill-Building (MHSB)

Mental Health Skill-Building (MHSB) is an individualized support program that encourages and empowers living independently in the community. It is a goal oriented service designed to enable individuals to achieve and maintain community stability and independence in the most appropriate, least restrictive environment. We help to increase individuals functional skills and appropriate behavior related to the individual’s health and safety, activities of daily living, and use of community resources.

 

With the help of our staff, clients will create a specialized treatment plan with goals specific to their needs. This service improves a client’s well-being by establishing a support network of safety, education and empowering the client to increase overall quality of life by managing and controlling symptoms. Our services include extensive collaboration with outside professionals.

At Prosperity, we are committed to providing quality mental health services to all. Although we specialize providing services to the LGBTQI+ community, our doors are open to EVERYONE. We are dedicated to the overall health of our clients and linking them to other community resources. We strive to create a safe space for each of our clients to be their authentic selves without fear of judgment, discrimination or trauma. In our office, we are intentional about making sure each client has what they need to feel safe to explore whatever is important to their life and sense of self. Our practice culture is centered on our key values: Transparency, Respect, and Open Communication. We’re looking forward to improving health care for diverse communities and ensuring that everyone feels welcome and included.
